Long-acting GLP-1 receptor agonist for type 2 diabetes (Ozempic/Rybelsus) and chronic weight management (Wegovy). Dosed once weekly subcutaneously.

Mechanism of action

Albumin-bound GLP-1 analog resistant to DPP-4 degradation. Enhances glucose-dependent insulin secretion, suppresses glucagon, slows gastric emptying, and reduces appetite via central GLP-1 receptors.

Boxed warning for thyroid C-cell tumors (rodent data). Titrate slowly to limit GI effects. Half-life ~1 week means ~5 weeks to reach steady state and ~5 weeks to wash out.
